Job summary

You will develop personalized care plans, educate families, and collaborate with care teams to transition across LTSS, with mileage reimbursement and a schedule Monday–Friday.

Qualifications

• Graduate from an Accredited School of Nursing with 4–6 years of related clinical nursing experience.
• BSN preferred and RN licensure in the state.
• Strong clinical documentation, communication, organization and problem-solving skills.

Responsibilities

• Evaluates the needs of complex/high risk members and develops care plans.
• Coordinates long-term care plans and collaborates with care management teams.
• Acts as liaison between members, families, physicians, and facilities.
• Monitors progress, revises plans, and ensures compliance with guidelines.
• Provides education on disease processes and available benefits.
