About This Role

The Senior Care Manager RN role involves assessing, planning, and implementing complex care management activities to ensure quality, cost-effective healthcare outcomes for members with physical health conditions and chronic disease management. This position is ideal for experienced RNs looking to make a significant impact in care coordination.

• Pay: $75,300 - $135,400/year

What You'll Do

• Develop and continuously assess ongoing care plans/service plans.
• Collaborate with providers to identify resources needed to address member's unmet needs.
• Coordinate and manage care between members, families, and care provider teams.
• Monitor care plans and member status, revising as necessary.
• Identify problems/barriers for care management and appropriate interventions.
• Review member data to identify trends and improve quality care.
• Develop appropriate care plans/service plans from referrals and intake assessments.
• Perform telephonic, digital, and home outreach to assess member needs.
• Collaborate with healthcare providers to facilitate member services.
• Provide education to members and their families on healthcare options and benefits.

Requirements

Education:

• Graduate of an Accredited School of Nursing
• BSN preferred

Experience:

• 4 – 6 years of clinical nursing, direct RN case management, and/or care coordination experience
• 7+ years of clinical nursing RN experience preferred

Licenses & Certifications:

• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
