Centene Corporation in Texas is seeking a Senior Care Manager RN to assess, plan, and implement complex care management activities for members with chronic conditions and preventive needs. You will develop personalized care plans, educate families, and collaborate with providers to ensure high-quality, cost-effective care.

Requirements include an RN license and 4–6 years of related experience, with strong communication and organizational skills.

Responsabilités

• Develops and continuously assesses ongoing care plans / service plans and collaborates with providers to identify providers, specialist, and/or community resources needed to address member's unmet needs
• Coordinates between the member and family/caregivers and the care team to ensure person-centered care or services
• Monitors care plans and status and revises goals to meet member needs
• Identifies problems/barriers for care management and interventions for escalated cases
• Reviews member data to identify trends and improve quality of care per regulations
• Reviews referrals and intake assessments to develop care plans
• May perform outreach via telephonic, digital, home or other sites to assess needs
• Collaborates with healthcare providers to facilitate services and revise care plans
• Collects and maintains member information to ensure compliance with regulations
• Educates members and families on disease processes, care options, and benefits
• Partners with leadership to improve care delivery cost-effectively
• Precepts clinical new hires and supports onboarding
• Provides guidance to new hires on navigating a Managed Care Organization
• Engages in onboarding readiness and cross-functional readiness measures
• Other duties as assigned and compliant with policies
