Job summary

Centene is hiring for a hybrid role in Central Texas to perform in-home assessments and lead care management for complex populations. The position focuses on coordinating services, developing personalized care plans, and ensuring high-quality, patient-centered care for members and families.

The nurse will evaluate needs, collaborate with providers, monitor plans, educate families, and act as an advocate while complying with state and federal guidelines.

Qualifications

• Graduate from an Accredited School of Nursing or a Bachelor's degree with 4–6 years of related experience.
• RN or NP licensure required; RUG certification must be obtained within 90 days of hire.

Responsibilities

• Assess care needs of high-risk members and develop long-term care plans.
• Coordinate with care team to identify providers and community resources.
• Educate members and caregivers on services, options, and benefits.
