Qualifications

• Graduate of an Accredited School of Nursing.
• BSN preferred.
• 4–6 years of clinical nursing, direct RN case management, and/or care coordination.
• 7+ years of clinical nursing RN experience preferred.
• RN licensure in state or compact state.

Responsibilities

• Develop and continuously assess care plans.
• Coordinate care between members, families, and providers.
• Monitor and revise care plans as needed.
• Provide education to members and families on healthcare options.
• Telephonic, digital, and home outreach to assess member needs.
• Collaborate with healthcare providers to facilitate member services.
