Job summary

Centene's Senior Care Manager RN role assesses, plans, and implements complex care management activities to develop personalized care plans for members with chronic conditions, guiding families on services and benefits to reach high-quality, cost-effective outcomes.

The position coordinates care across providers, monitors progress, approves revisions to plans, and supports onboarding of new hires while aligning with state and federal regulations and Centene's comprehensive benefits and flexible

Qualifications

• RN with a nursing degree and 4–6 years of related experience.
• Active RN license in state or compact (as applicable).
• Preferred: 7+ years of clinical RN experience with direct patient care and care coordination.

Responsibilities

• Develops and continuously assesses care plans and coordinates with providers to identify needed resources.
• Coordinates between member, family/caregivers, and care team to ensure person-centered care.
• Monitors progress and revises care plans to meet member goals.
• Identifies barriers to care and applies appropriate interventions.
• Reviews member data to improve quality and regulatory compliance.
• Performs outreach to assess needs and connect to resources.
• Collaborates with providers to adjust care or services as needed.
• Documents and maintains all member information per regulations.
• Provides education to members and families on disease processes and benefits.
• Partners with leadership to improve care delivery costs.
• May precept new hires and support onboarding and readiness.
