Job summary

Commonwealth Senior Living is seeking a Resident Care Director to lead the nursing team and coordinate comprehensive resident care within the community. This role partners with physicians, pharmacists, and care providers to ensure integrated service delivery and regulatory compliance.

The RCD oversees assessments, care planning, staffing, and budget, while driving quality and safety in patient care. The position requires RN licensure, nursing management experience, and strong communication

Qualifications

• Minimum of two years’ college in human services or health care field.
• Holds an unencumbered state license as a professional Registered Nurse.
• Minimum of two years’ experience providing resident care to the elderly or memory care.
• Minimum of one year nursing management experience (hiring, coaching, scheduling).
• Experience with resident assessment and care planning.
• CPR and First Aid certification.
• Willing to become trained in state assessments/service plans per licensing.
• Knowledge of infection control practices.
• Strong verbal and written communication skills.
• Ability to show compassion to cognitively impaired older adults.
• Ability to work weekends/evenings/flexible hours; on-call responsibilities.
• Proficient with basic office equipment and computer use.
• Valid driving record.

Responsibilities

• Model a customer-centric approach to residents and families.
• Promote health and wellness through coordinated care with physicians and providers.
• Ensure compliance with Federal and State policies governing resident care.
• Collaborate to achieve occupancy goals and referral development.
• Establish rapport with residents, families, and regulatory personnel.
• Conduct routine resident care rounds and address concerns.
• Ensure regular assessments and updates to care plans.
• Monitor resident care quality data and staffing budgets.
• Oversee education and training for direct care staff.
• Manage medication safety and the care team’s performance.
