Job summary

Commonwealth Senior Living is seeking a Resident Care Director (RN or LPN) to lead the resident care team and coordinate care delivery for our community. This full-time role partners with hospitals, physicians, and health agencies to ensure high-quality, compliant resident care and service delivery.

You will model our core values, supervise care staff, manage care plans, and ensure regulatory licensing is met. Weekend/evening/on-call flexibility is required.

Qualifications

• RN or LPN license, unencumbered
• Minimum of two years’ college in health care or human services
• Minimum of one year nursing management experience
• Experience with resident assessment and care planning
• CPR and first aid certification
• Willingness to be trained in state assessments and service plans
• Knowledge of infection control and safety practices
• Strong communication skills, written and verbal

Responsibilities

• Lead and manage the resident care team and coordinate care delivery.
• Collaborate with physicians, hospitals, and agencies to optimize resident care.
• Ensure compliance with Federal, State, and Board licensing requirements.
• Oversee care programs, performance management, and regulatory adherence.
• Monitor and improve resident care quality data and outcomes.
• Schedule and staffing planning to meet resident needs and budgets.
• Supervise direct care staff and conduct training and orientation.
• Maintain resident safety and medication management oversight.
• Engage with residents and families, ensuring compassionate support.
• Coordinate with administration for occupancy and referrals.
