Job summary

Commonwealth Senior Living at Cedar Bluff in Salisbury, MD seeks a Resident Care Director to lead the resident care team and coordinate care and service delivery for residents. The role liaises with hospitals, physicians, and care providers to ensure optimal, compliant care while modeling the company’s core values.

The RCD will oversee care programs, performance management, and regulatory compliance, and will guide staffing, assessments, and care planning.

Qualifications

• Two years of college in health care or related field.
• Minimum of two years’ experience providing resident care to the elderly or similar memory care.
• Minimum of one year nursing management experience including hiring, coaching, and scheduling.
• Experience with resident assessment and care planning.
• CPR and First Aid certification.
• Willingness to become trained in state assessments and service plans as required.
• Knowledge of infection control practices and prevention of disease transmission.
• Strong verbal and written communication skills.
• Empathy for cognitively impaired residents and families.

Responsibilities

• Lead the resident care team and coordinate care delivery.
• Collaborate with physicians, hospitals, and care providers to promote integrated care.
• Ensure compliance with Federal/State policies and licensing.
• Support occupancy goals through referral development and liaison activities.
• Oversee staff scheduling, performance, and education initiatives.
• Conduct resident assessments and maintain individualized care plans.
• Monitor care quality data and identify improvement opportunities.
• Maintain safe working environment and ensure proper use of company policies.
