Acentra Health is seeking a Senior Program Director to lead the new CA UIS Care Coordination Program, guiding design and operations across California. The role requires strategic and operational leadership, coordinating interdisciplinary teams and partners to deliver timely, equitable, and person-centered care.

The position is remote, but candidates must reside in California, with preferred locations in LA or Sacramento.

Qualifications

• Bachelor's Degree in healthcare, nursing, social work or related field.
• 8+ years' experience in case management and care coordination.
• 8+ years experience in healthcare operations, or CA public sector health and human services related programs, preferably in Medi-Cal or MCO environments
• 10+ years experience in leadership/management roles, and engaging stakeholder relationships.
• Proven Leadership skills and ability to influence beyond this role; strong ability to build credibility and relationships internally and externally.
• Proven track record to meet and enforce deadlines, and conduct research into issues
• Must reside in California, but position is remote
• Flexibility to travel up to 25%.

Responsabilités

• Direct program through objectives and contract compliance. Monitors metrics and daily operations to ensure success.
• Collaborate with program leaders, including medical director, management teams, and corporate support teams to improve processes, determine training needs and improvement opportunities.
• Engage community partners and stakeholders in delivering member outreach
• Provide leadership and supervision to staff assigned to the program, including quality, Case Management, Care Coordination, provider training/education, consumer education, data collection, analysis and reporting, and technical assistance, to ensure compliance with accreditation standards.
• Works with management and corporate team for the enhancement of the contract scope, new RFP cycles and related new business.
• Oversees budget review and analysis to ensure the financial success of the program.
• Assesses all systems, including contract, policy and procedure, workflow, etc., continually to ensure a high level of customer satisfaction to all stakeholders.
• Collaborates with management team to create an organizational structure that will ensure effective processes and work designs, including team integration and oversight which includes California field-based outreach and localized community engagement, within the southern region.
• Assists in determining professional development opportunities for staff and teams.
• Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.
