Acentra Health is seeking a Senior Program Director to lead a new CA UIS Care Coordination program across the state. You will provide strategic and operational leadership, overseeing design, day-to-day operations, workforce performance, quality outcomes, regulatory compliance, and service delivery.

The role partners with clinical, operational, provider, health plan, and community organizations to ensure timely, equitable, and person-centered care.

Qualifications

• Bachelor’s degree in healthcare, nursing, social work or related field.
• 8+ years in case management and care coordination.
• 8+ years in healthcare operations or CA public sector health and human services related programs.
• 10+ years in leadership/management roles with stakeholder engagement.
• Must reside in California; remote role.

Responsibilities

• Direct program through contract objectives and monitor metrics.
• Lead collaboration with medical director and management teams.
• Oversee care coordination workflows and transitions of care.
• Ensure regulatory compliance and accreditation standards.
• Manage budget and financial performance.
• Develop organizational structure for field outreach in the southern region.
