Job summary

Acentra Health is seeking a Senior Program Director to lead a new CA UIS Care Coordination Program from a remote location, with residency in California preferred.

You will provide strategic and operational leadership across interdisciplinary teams, manage care coordination workflows, strengthen transitions of care, and monitor access, engagement and outcomes for California members. Travel up to 25% may be required.

Qualifications

• Bachelor’s Degree in healthcare, nursing, social work or related field.
• 8+ years’ experience in case management and care coordination.
• 8+ years experience in healthcare operations, or CA public sector health and human services related programs, preferably in Medi-Cal or MCO environments
• 10+ years experience in leadership/management roles, and engaging stakeholder relationships.

Responsibilities

• Direct program through objectives and contract compliance; monitor metrics and daily operations.
• Collaborate with medical director, management teams, and corporate support to improve processes.
• Engage community partners and stakeholders in member outreach.
• Provide leadership and supervision to staff across quality, care coordination, provider training/education, consumer education, data collection, analysis and reporting, and technical assistance, to ensure compliance with accreditation standards.
• Oversee budget review and ensure the financial success of the program.
