Job summary

Acentra Health is seeking a Senior Program Director to lead a new CA UIS Care Coordination Program, coordinating care across California with a focus on timely, equitable, and person-centered services.

You will drive program design, day-to-day operations, workforce performance, and regulatory compliance, partnering with clinical, provider, health plan, and community organizations to ensure high-quality outcomes.

Qualifications

• Bachelor’s degree in healthcare, nursing, social work or related field.
• 8+ years’ experience in case management and care coordination.
• 8+ years experience in healthcare operations, or CA public sector health and human services related programs, preferably in Medi-Cal or MCO environments.
• 10+ years experience in leadership/management roles, and engaging stakeholder relationships.

Responsibilities

• Direct program through objectives and goals for contract compliance; monitor metrics and daily operations.
• Collaborate with program leaders to improve processes, identify training needs and improvement opportunities.
• Engage community partners and stakeholders in delivering member outreach.
• Provide leadership and supervision to staff, ensuring compliance with accreditation standards.
• Oversee budget review and analysis for financial success of the program.
• Ensure systems and policies support high customer satisfaction for stakeholders.
• Coordinate California field-based outreach and localized community engagement within the southern region.
