Qualifications

• Active, unrestricted RN license (state or compact).
• 7+ years in nursing or healthcare leadership.
• 5+ years in healthcare management or team leadership.
• Strong verbal and written communication.
• Ability to travel up to 25% within CA.
• Proficiency with Microsoft Office and care coordination software.

Responsibilities

• Provide overall clinical and operational leadership for Care Coordination and Discharge Case Management activities.
• Direct, supervise, and evaluate program staff to ensure quality and consistency.
• Ensure program deliverables and contract requirements are met with high standards.
• Monitor KPIs, productivity, and outcomes; implement improvements as needed.
• Collaborate with senior leadership to identify opportunities for process improvement.
• Lead cross-functional committees and advise on clinical operations and policy.
