Acentra Health is seeking a Clinical Manager (RN) to provide clinical and operational leadership for the CA UIS Care Coordination and Case Management programs. You will oversee performance, ensure clinical quality, and drive contract delivery while guiding clinical staff and coordinating care.

This remote role within California requires strong leadership and communication skills, with travel up to 25% within CA; preference for Sacramento, Los Angeles, or San Diego locations.

Qualifications

• Active, unrestricted RN license (or compact license).
• Associate degree, OR 7 years equivalent experience in nursing or related field.
• Minimum 5+ years healthcare management, supervisory, or leadership experience.
• Strong verbal and written communication skills.
• Customer-focused, results-oriented, able to build internal/external relationships.
• Organizational skills with ability to handle multiple assignments.
• Proficiency in Microsoft Office and healthcare software.
• Travel up to 25% within CA as needed.

Responsibilities

• Provide clinical and operational leadership for Care Coordination and Discharge Case Management activities.
• Direct, evaluate, and coach program activities to ensure clinical quality and contract performance.
• Oversee Case Managers and Care Coordinators, including performance management and accountability.
• Ensure deliverables, workflows, and contract requirements are consistently achieved.
• Monitor KPIs, productivity, quality metrics, and program outcomes with improvement actions.
• Oversee member outreach, engagement, care coordination, and transition-of-care activities.
• Serve as Clinical SME and provide guidance on complex issues.
• Collaborate with senior leadership to identify process improvement opportunities.
• Lead cross-functional initiatives and committees, fostering collaboration.
• Ensure adherence to clinical standards, regulations, and company policies.
