Job summary

Oak Street Health is hiring a Primary Care Physician to deliver equitable value-based care to local Medicare populations across our neighborhood clinics. You will work with large care teams to focus on outcomes and preventive medicine rather than volume.

Qualifications

• Must hold MD or DO.
• Board certification in Internal Medicine or Family Medicine or be eligible.
• Active, unrestricted State License to practice.
• Active DEA license and US work authorization.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
