Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations across our neighborhood clinics. You will work at a single medical center with large care teams to focus on quality of care over volume, with bonuses tied to outcomes and preventive care.

Ideal candidates are motivated to provide excellent care to older adults; new graduates are welcome. Center Medical Director positions may be considered for highly qualified applicants.

Qualifications

• MD or DO graduate required.
• Board certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State License and DEA license required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage medications and treatments.
• Maintain EMR data in canopy and Greenway systems.
• Collaborate with regional/central leadership to meet quality goals.
• Direct and manage the care team with Practice Managers.
