Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care for local Medicare populations across our neighborhood clinics. You will work with large care teams to focus on outcomes and preventive care in a supportive, collaborative environment.

The role welcomes new graduates and offers a pathway to leadership positions, with competitive bonuses and a strong focus on patient experience. A full-time schedule with standard daytime hours is provided.

Qualifikationen

• MD or DO degree and board certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State License in the US.
• Active DEA license.

Aufgaben

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
