Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care for Medicare patients at our neighborhood clinics. You will collaborate with a large care team to emphasize outcomes and preventive care over service volume.

Qualifications

  • Must hold an MD or DO degree.
  • Board certification in Internal or Family Medicine or be eligible.
  • Active, unrestricted state medical license.
  • Active DEA registration.
  • U.S. work authorization.
  • Geriatrics fellowship preferred.
  • Experience with geriatrics.
  • Experience with value-based care in outpatient settings.
  • Outpatient primary care experience desired.

Responsibilities

  • Assess and diagnose patients at local clinics.
  • Oversee, direct, and administer primary care.
  • Prescribe and manage medications and therapies.
  • Maintain patient EMR data via Canopy and Greenway.
  • Collaborate with regional and central leadership to meet quality goals.
  • Work with Practice Managers to direct and manage the center care team.
  • Other duties as assigned.
