Oak Street Health seeks a Primary Care Physician to deliver equitable, value-based care to local Medicare populations within our neighborhood centers. You will operate within a large care team focused on quality outcomes and patient experience.

Qualifications

• MD or DO degree required and board certified or eligible in Internal/Family Medicine.
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization to practice medicine in the United States.

Responsibilities

• Assess, diagnose, and treat patients at local clinics.
• Oversee and administer primary care within the center network.
• Prescribe and manage pharmacologic treatments and medications.
• Maintain Electronic Medical Records accurately.
• Collaborate with leadership to meet health quality goals.
• Partner with practice managers to guide the center care team.
• Perform other duties as assigned.
