Job summary

Oak Street Health is hiring a Primary Care Physician in the Chicago area to deliver equitable, value-based care to Medicare patients across a network of neighborhood clinics. Qualifications

• MD or DO degree and board certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State Medical License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer medications.
• Maintain Patient Electronic Medical Record data via EMR systems.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
