Job summary

Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based healthcare to local Medicare populations across our network of neighbourhood primary care centres. The role focuses on delivering high-quality care in a team-based setting, supported by performance bonuses and care coordination.

Qualifications

• MD or DO degree from an accredited medical school (osteopathic or allopathic).
• Board Certification in Internal Medicine or Family Medicine (or eligible).
• Active, unrestricted State License to practice medicine.
• Active DEA licence.
• US work authorisation.

Responsibilities

• Assess and diagnose patients at our local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data via canopy and greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the centre care team
• Other duties as assigned
