Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations across our neighborhood clinics. You will work with large care teams to focus on outcomes, preventive care, and patient experience.

Qualifications

• MD or DO with valid US medical license.
• Board Certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State License.
• Active DEA license.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient EMR data via Canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
