Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care for Medicare patients in our neighborhood centers. You’ll work with a supportive team to focus on quality over volume and improve patient experiences.

Qualifications

• MD/DO degree required or equivalent.
• Board Certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State License required.
• Active DEA license required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data via Canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
