Job summary

FIVCO ADD is seeking a Meals Specialist to link clients with meal-based services, conduct home visits, and perform case management per care plans. Under supervision, you will coordinate with agencies and document services, ensuring compliance with federal and state guidelines while supporting rural communities.

You will assess eligibility for Title III Home Delivered Meals, manage cases, and assist clients in obtaining additional services.

Qualifications

• Bachelor’s degree in one of the specified fields.
• Experience working with elderly populations encouraged.
• Knowledge of federal and state aging/disability policies.

Responsibilities

• Provide initial assessment and annual reassessment for Title III Home Delivered Meal Program eligibility.
• Case manage individuals receiving home-delivered meals and assist Homecare clients.
• Coordinate services from agencies per Plan of Care.
• Maintain client case files with interactions.
• Ensure staff understand the Plan of Care and compliance procedures.
• Assist clients in applying for additional services or benefits.
• Document all client contacts and services in records.
• Travel to meetings and trainings, including overnight stays.
• Perform other duties as assigned.

Bachelor’s degree in Social Work/ Gerontology/ Psychology/ Sociology or geriatrics-related field

Master’s degree in human services (substitutes for experience)

Kentucky-registered nurse

Licensed practical nurse with Kentucky license + 3 years elderly experience
