Job summary

Meals on Wheels Western Michigan is seeking an In-Home Care Services Coordinator to assess older adults in-home and by phone, ensuring program eligibility and appropriate service levels. You will report to the Home Delivered Meals Program Manager and support clients in navigating services and resources.

The role emphasizes aligning with the organization’s mission, maintaining client records, and coordinating referrals while working mainly remotely with some in-office meetings.

Qualifications

• Strong time-management skills and the ability to independently manage competing priorities.
• Exceptional communication skills, both written and verbal, for engaging clients, team members, and partners.
• Valid driver’s license with reliable transportation and willingness to travel within the service area.
• Acceptable criminal background.

Responsibilities

• Align with mission and values of Meals on Wheels Western Michigan.
• Conduct in-home and phone assessments to determine eligibility and service levels.
• Maintain electronic client records and document assessment information.
• Assist clients with program navigation and make referrals to community resources.
• Report suspected or confirmed abuse, neglect, or exploitation.
• Work primarily remotely with some in-office meetings and trainings.
