Job summary

FIVCO Area Development District is seeking a Meals Specialist to link clients to meal-based services under the Social Services division. You will perform home visits and required calls to support plans of care, coordinating with agencies and ensuring eligibility and service delivery.

The role involves travel within rural areas, data entry in the reporting system, and assisting clients in accessing other benefits while maintaining compliance with procedures.

Qualifications

• Must have a bachelor's degree in one of the listed fields or nursing with Kentucky license.
• Experience with elderly care and home-delivered meals preferred.
• Knowledge of aging resources and community services required.

Responsibilities

• Assess eligibility for home-delivered meal program and reassess annually.
• Case manage and coordinate services across multiple agencies.
• Maintain client files and document all contacts and plans of care.
• Travel for home visits, meetings, and training; work under supervision.
