Job summary

FIVCO Area Development District seeks a Caregiver Coordinator in the Social Services division. Under the Associate Director of Social Services, you will manage initial assessments, case management, and coordination of caregiver programs for older adults and people with disabilities.

The role requires strong organizational and communication skills and ability to work with volunteers. A high school diploma and relevant experience are preferred; a valid driver’s license is required.

Qualifications

• High school diploma or equivalent; preferred three years of direct experience working with the elderly or disabled.

Responsibilities

• Assist with pre-screening and intake to connect the consumer to appropriate services.
• Facilitate program enrollment for publicly funded services.
• Record all contacts into the data system at the time of contact and ensure data completeness.
• Maintain a computerized client tracking system and secure case files.
• Assist in placing consumers on a waitlist within the state data system and internal records.
• Process program vouchers used for respite and supplemental services.
• Coordinate with grandparents approved to receive supplemental services to shop for approved supplies.
• Attend job development training and staff meetings as requested.
• Respond to calls from the public and provider agencies regarding programs, data, and resources.
• Identify service needs, resources and barriers for older adults and persons with disabilities.
• Conduct home visits at enrollment and recertification as needed.
• Coordinate support groups, health fairs, caregiver events and outreach activities.
• Understand the Older Americans Act and state policies; track caregiver funds.
