National Church Residences in Greenfield, Indiana, is seeking a Service Coordinator to advocate for residents and coordinate programs that improve quality of life. You will liaise with community agencies, identify services, and develop referral streams to support elderly residents and their families.

The role emphasizes problem-solving, outreach, and collaboration with local providers to deliver measurable outcomes and compassionate resident care.

Formación

• Bachelor’s degree in social work or a related field preferred.
• Two or more years of experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsabilidades

• Advocate, organize, problem-solve, and deliver results for residents.
• Coordinate programs and activities for residents.
• Serve as a liaison to community agencies, network with community service providers, and seek out new services.
• Engage residents to identify needs and refer to community agencies.
• Develop a Resource Directory of state/local service providers.
• Sponsor educational events on health care, agency support, life skills and referrals.
• Provide quality customer service to residents and assist with basic needs.
• Assist residents in obtaining community services such as housekeeping, meals, transportation, personal services, and financial assistance.
• Monitor and relay feedback from residents and families.
• Manage relationships with internal and external partners using effective communication.
