Qualifications

• High School Diploma or GED required. A bachelor’s degree in social work or a related field is preferred.
• Two or more years of experience in a social service delivery with elderly, people with disabilities, and/or family population.
• Working knowledge of Windows, Microsoft Office Suite and World Wide Web.

Responsibilities

• Coordinate programs and activities for eligible individuals.
• Serve as a liaison to community agencies and seek services for participants.
• Monitor and document service provision in the management system.
• Develop and maintain a Resource Directory of state/local service providers.
• Organize educational events and promote participation.
• Identify partners to provide health services and screenings at home or in community settings.
• Assist participants in building informal support networks.
