Job summary

National Church Residences is seeking a Service Coordinator I to coordinate programs and services for seniors in housing communities. Under supervision, you will act as liaison to community agencies and help participants maintain quality of life and age in place.

You will conduct assessments, organize health-related education events, document interventions, and build informal support networks for residents. Strong communication and computer skills are essential.

Qualifications

• High School Diploma or GED required; a bachelor's degree in social work or related field preferred.
• Two+ years of experience in social services serving elderly or disabled populations.

Responsibilities

• Coordinate programs and activities for eligible individuals and act as liaison to community agencies.
• Monitor services provided by partners and document participant usage in the system.
• Develop and maintain a Resource Directory of local service providers.
• Organize health-related education events and promote participation by participants.
