National Church Residences is seeking Service Coordinators to support seniors by coordinating programs, advocating for residents, and connecting them to community services. This part-time role emphasizes teamwork and delivering measurable outcomes in a housing and care setting.

The position requires a Bachelor’s degree in social work or related field and 2+ years of experience in social services for elderly or families. A commitment to resident well-being and strong communication are essential.

Qualifications

• Education: Bachelor’s degree in social work or a related field preferred.
• Experience: Two or more years of experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.

Responsibilities

• Advocate, organize, problem-solve, and deliver results for residents.
• Coordinate programs and activities for residents.
• Serve as liaison to community agencies and network with service providers.
• Identify low-cost service providers and negotiate discounts.
• Engage residents to identify needs and refer to community services.
• Develop a Resource Directory with state/local providers.
• Sponsor educational events on health care, agency support, life skills, and referrals.
• Provide quality customer service to residents and assist with basic needs.
• Assist residents in obtaining community services like housekeeping, meals, transportation, personal services, financial aid, day care counseling.
• Monitor and relay feedback from residents and families.
• Manage relationships with internal and external stakeholders.
