Goodman Jewish Family Services (GJFS) Broward County is seeking a compassionate Case Manager to support older adults in the Senior Housing Stability Project. You will deliver comprehensive case management, assess needs, coordinate housing stabilization, and connect clients with financial assistance and community resources.

Working in collaboration with Coast to Coast Legal Aid of South Florida, you will advocate for clients, navigate benefits, and help develop personalized plans to promote aging

Formación

• Bachelor's degree in social services or related field required; Master’s degree preferred.

Responsabilidades

• Conduct comprehensive assessments and develop individualized service plans with measurable goals.
• Monitor client progress and adjust plans as needed to address changing needs.
• Coordinate housing-related financial assistance and connection to benefits.
• Advocate for clients and facilitate referrals to internal and partner programs.
• Collaborate with landlords, housing providers, and community partners to prevent displacement.
• Maintain compliant and timely documentation of services and outcomes.
