Job summary

Alpert Jewish Family Service is seeking a guardianship case manager to coordinate intake, assessments, and service plans for wards appointed by the courts. You will conduct bio-psychosocial evaluations in clients’ homes or other settings and collaborate with Guardianship teams to ensure appropriate care and reporting.

You will attend hearings, communicate with attorneys, review financial documents, and maintain case records to meet quality standards under Florida law.

Qualifications

• Graduate degree from an accredited School of Social Work or other Behavioral Mental Health program.
• 3-5 years’ experience in a similar role.
• Experience with geriatric and special populations.
• Knowledge of MS Office Suite (Word, Excel) and familiarity with ERM systems.
• Valid Florida driver’s license with good driving record; automobile insurance proof.

Responsibilities

• Coordinate intake, evaluation, implementation, and provision of services with proper documentation.
• Attend court sessions as required and communicate with Guardianship attorneys.
• Maintain case records and prepare reports for court renewal of guardianship.
• Perform bio-psychosocial assessments in client homes or other settings.
• Develop service plans based on assessments and established working relationships.
• Visit wards and family members to gather information and facilitate services.
• Intervene to help wards achieve functional independence and appropriate levels of care.
• Coordinate services with professionals, vendors, and funders in the community.
