Job summary

Gulfstream Goodwill Industries in West Palm Beach, FL is seeking a housing-focused case manager to work at the Lewis Center. You will partner with participants to develop individualized housing plans, provide guidance, and advocate to maximize independence in the community.

Responsibilities include intake review, SPDATs, coordinating resources, data entry, and collaboration with staff and community partners to remove housing barriers. A valid FL driver’s license is required.

Qualifications

• Bachelor’s degree required in a related field.
• Minimum of one year case management experience preferred.
• Excellent communication skills required and ability to establish rapport with employers and referral agents.
• Excellent problem-solving and organizational skills.
• Valid Florida driver’s license, insurance, reliable transportation for travel to outlying locations.

Responsibilities

• Provide housing-centered case management for individuals experiencing homelessness.
• Develop individualized housing plans and assist with housing options in the community.
• Conduct intake reviews, SPDATs, and assess barriers to housing.
• Coordinate transportation to appointments and services as needed.
• Enter participant data into database systems and ensure consents are on file.
• Collaborate with program providers, staff, and community resources to support housing goals.
• Attend meetings and participate in routine trainings and program changes.
