Job summary

Gulfstream Goodwill Industries seeks a housing-focused case manager for the Lewis Center in West Palm Beach. You will work with participants to create Individualized Housing Plans, advise on benefits, and coordinate with service providers to expand housing opportunities.

You will conduct SPDATs, track progress, provide trauma-informed support, and assist with transportation, documentation, and data entry. The role requires collaboration with staff and community partners to empower clients toward

Qualifications

• Bachelor’s degree in Rehabilitation, Social Services, Human Services, or related field.
• Minimum of one year of case management experience preferred.
• Excellent communication skills and ability to build rapport with partners.
• Strong problem-solving and organizational skills.
• Must have a valid Florida driver's license and transportation for travel.

Responsibilities

• Provide housing-focused, person-centered case management for individuals experiencing homelessness.
• Develop individualized housing plans and assist participants to meet housing goals.
• Review SPDATs and coordinate with participants to set housing goals.
• Coordinate with program providers, staff, and partners; ensure data entry into databases.
• Advise on resources and referrals and assist with applications for benefits.
• Attend staff meetings and trainings; participate in program enhancements.
• Assist with transportation, clothing, and crisis intervention as needed.
