Job summary

Jewish Community Services of South Florida (JCS) is seeking a Case Manager to provide intake, assessment, and ongoing case management for elderly clients. The role includes developing service plans, coordinating referrals, and maintaining compassionate support for families.

Responsibilities cover in-person visits, crisis intervention, documentation, and resource connections. The team emphasizes compassionate care, ethics, and collaboration with local providers to promote well-being.

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• Minimum 2 years of relevant experience preferred.
• LCSW, LMFT, LMHC license preferred or license eligible.
• Ability to speak fluent Spanish, Yiddish, or Russian is a plus.
• Must have a valid Florida Driver’s License with proof of insurance and ability to travel within Miami-Dade County.

Responsibilities

• Complete assessments and program documents approved by the Department Director.
• Monthly in-person home visits and phone check-ins as required.
• Development of Case Management Treatment Service plan.
• Crisis intervention.
• Information and referrals to community resources.
• Arrange for client services as needed.
• Data entry into a web-based software system.
• Work performed through home visits, Kendall Office, and HQ office as requested.
