Job summary

Jewish Family Children's Services of the Suncoast is seeking an Aging Services Case Manager to provide strengths-based case management and supportive counseling to older adults and their families. You will help clients navigate systems, build coping skills, and access resources in in-office, in-home, and community settings.

The role emphasizes collaboration with interdisciplinary teams, documentation, and ongoing assessment to maintain safety, dignity, and independence for aging clients.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services or related field.
• Minimum of two years’ experience working with the aging population.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Provide case management services to clients within the Aging Services program.
• Assess client and family strengths, challenges, and needs.
• Develop, write, implement, evaluate, and revise treatment and service plans based on ongoing assessments and client/family input.
• Conduct ongoing assessment of behavior, mental status, signs of mental health disorder, physical health, substance use, housing, finances, and family functioning.
• Refer and link clients and families to appropriate internal and external resources.
• Advocate for client needs and liaison with community agencies to coordinate treatment.
• Provide supportive and educational counseling (individual, family, and group).
• Support clients in building natural support systems to improve stability and reduce isolation.
• Participate in interdisciplinary treatment team meetings and care planning.
• Maintain accurate documentation, progress notes and assessments, per agency and funder requirements.
• Complete required trainings and other duties as assigned.
