Job summary

InCommunity is seeking a Direct Support Professional II (DSP II) to provide person-centered support to individuals with intellectual and developmental disabilities in residential and community settings.

The DSP II will assist with ADLs, medications, health monitoring, documentation, and coordination with an interdisciplinary team. A valid driver’s license and CPR/First Aid certification are required, with pathways to certification as DSP II through approved programs.

Qualifications

• High school diploma or GED required.
• Two years direct support experience required; three to five years preferred.
• Valid driver’s license and evidence of insurance coverage.
• CPR/First Aid and Medication Administration certification required or achievable within timeframe.
• DSP II Certification through a recognized program within timeframe.

Responsibilities

• Provide person-centered services that promote choice, dignity, independence, and community inclusion.
• Assist with ISPs, Behavior Support Plans, and risk mitigation strategies.
• Teach and assist individuals with ADLs and life skills to promote independence.
• Provide one-to-one supports per approved service plans.
• Encourage participation in community activities reflecting individual interests and goals.
• Administer medications and supports per regulations and organizational policies.
• Monitor health, safety, and well-being; report concerns promptly.
• Transport and accompany to medical appointments; document updates within 24 hours.
• Maintain a safe, clean home environment; assist with meals and nutrition.
• Complete required documentation accurately and within timeframes.
• Ensure compliance with DBHDD, CARF, Medicaid and InCommunity policies.
• Lead and mentor DSP staff; participate in onboarding and coaching.
