Job summary

Qualifications

• High School Diploma or GED required.
• Associate's or Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred.

Responsibilities

• Supervise and support individuals per their Person-Centered Support Plan.
• Assist with activities of daily living (ADLs) and transportation.
• Administer medications per agency policies and training.
• Complete required documentation in a timely manner.
• Communicate changes in an individual's status to supervisors and clinical staff.
• Participate in required trainings, team meetings, and planning activities.
