Job summary

Bridgewell in Massachusetts provides an array of day habilitation programs and community-based supports that empower people with disabilities to live independent lives. The Service Manager 3 will supervise Level 3 day habilitation staff and implement individualized programs to meet each participant’s needs in daily living skills, community participation, and behavioral support.

This full-time role requires collaboration with DDS, DMH, MassHealth, HUD, DPH, and CARF; overseeing assessments,

Qualifications

• Associates Degree in Human Services or equivalent experience.
• A minimum of one year of experience working with people with developmental or psychiatric disabilities.
• Training and/or experience in autism and/or behavioral support.

Responsibilities

• Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF and Bridgewell policies.
• Administer and interpret assessments to determine each individual's skill level annually.
• Plan and implement an individualized program based on each individual's needs following their Day Habilitation Support Plan.
• Teach skills to individuals and small groups for training in designated area.
• Develop adaptive equipment and jigs to ensure individuals attain goals.
• Supervise the Day Habilitation Support Plan and maintain individual records in TIER.
• Assist people in daily routine decisions and access to communication with family and others.
• Support participation in community events including volunteer sites and libraries.
• Maintain certifications including FA/CPR and PABC.
• Other duties as assigned.
