Job summary

Bridgewell in Massachusetts seeks a Service Manager 3 to provide support in Level 3 day habilitation programs. You will assist individuals requiring daily intervention, supervision, and skills training in daily living activities within a day program and community settings.

The role involves creating and supervising individualized programs, addressing diverse needs, and maintaining records with compliance to regulatory standards.

Qualifications

• Knowledge and understanding of normalization, behavior management, and human rights.
• Excellent written and oral communication to prepare clear documents.
• Excellent organizational and problem-solving skills.
• Valid driver’s license and driving experience.
• Computer literacy to use Microsoft products and web-based applications.

Responsibilities

• Provide services to individuals according to applicable standards and Bridgewell policies.
• Administer and interpret assessments to determine skill levels annually.
• Plan and implement individualized programs based on each person’s needs.
• Teach skills to individuals and small groups.
• Develop adaptive equipment to help individuals reach goals.
• Supervise Day Habilitation Support Plan implementation and maintain records.
• Assist people in daily routines and ensure access to communication with others.
• Support participation in community events and volunteer sites.
• Maintain certifications including FA/CPR and PABC.
