Job summary

ChenMed is seeking a Primary Care Physician (PCP) to deliver comprehensive care to an older adult patient panel, up to 450 patients. The PCP will lead a care team, coordinate cross-functional care, and maintain high-quality outcomes while collaborating with hospitalists and APPs.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Must obtain/hold State Medical License for practiced state(s).
• Board certification in Internal Medicine, Family Medicine, Geriatrics or similar is preferred; Board Eligibility required.
• Must have current DEA number for controlled substances.
• BLS certification within first 90 days of employment.

Responsibilities

• Provide care for older adult patients with acute and chronic illnesses.
• Take accountability for patient care and outcomes, consulting with specialists as needed.
• Engage hospitalists when patients are admitted.
• Manage assessment, diagnosis, treatment, education, health promotion and care coordination.
• Lead care team including medical assistant, care facilitator, and care coordinator.
• Coordinate care for patients unable to come to office (in hospital, SNF, LTC, homebound).
• Recruit patients and providers as needed; cover for others and supervise APPs.
• Collaborate with APPs and remain accountable for their actions.
• Perform other duties as assigned by management.
