Qualifications

• Board certification is preferred; board eligibility required.
• Active DEA number for controlled substances.
• BLS certification required within 90 days of employment.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Take accountability for patient outcomes and coordinate with specialists as needed.
• Collaborate with hospitalists and care teams to ensure continuity of care.
• Lead the care team including medical assistant, care facilitator, and care coordinator.
• Engage in patient recruitment and center development as needed.
• Supervise APPs and ensure compliance with policies and guidelines.
