Qualifications

• 2 years of experience in referral management or care coordination.
• Ability to work with diverse populations including staff, providers, members, and families.
• Understanding of Medicare criteria for equipment and supplies (benefits/coverage).
• Basic medical terminology and CPT/ICD-10 knowledge.

Responsibilities

• Partner with the care team to understand each member's needs and identify gaps in treatment.
• Coordinate referrals to external providers and services to meet care needs.
• Document member referrals to keep records accurate and current.
• Build relationships with members and families to facilitate care coordination.
• Monitor progress on care plans and follow up on goals.
• Adhere to HIPAA standards and protect PHI.
• Facilitate clear communication among care team and clinicians.
• Assist members with barriers related to health care, finances, or transportation.
