Qualifications
- 2 years in referral management or care coordination
- Experience with EMR systems (preferably eClinicalWorks)
- Medical terminology knowledge and HIPAA awareness
Responsibilities
- Partner with multi-disciplinary care team to understand member care needs
- Coordinate referrals to external providers and services
- Document referrals accurately and keep records current
- Build relationships with members and families to facilitate care coordination
- Monitor care plan progress and follow up on goals
- Adhere to HIPAA and protect PHI
- Facilitate clear communication among care team and clinicians
- Assist members with barriers related to health care, finances, or transportation
