ArchWell Health seeks a Referral Coordinator to support seniors in navigating the healthcare system and ensuring timely referrals to the right specialists, inside or outside centers. The role emphasizes attention to detail and a passion for quality senior care.

Ideal candidates will understand Medicare criteria, coordinate with multidisciplinary teams, and maintain accurate documentation while upholding HIPAA standards. Strong Spanish-language skills are a plus for the communities served.

Qualifications

• Requires understanding of healthcare ecosystem and insurance authorization processes.
• Experience in care coordination or referral management is preferred.
• Spanish language skills are a plus for service area communities.

Responsabilités

• Coordinate referrals to internal and external providers to meet member care needs.
• Document referrals accurately for member records and care plans.
• Serve as a liaison between members, families, and care team to facilitate timely care.
