Qualifications

• Deep understanding of the healthcare ecosystem and insurance/authorization processes.
• Fluency in Spanish or languages spoken by the community served where necessary.
• Strong communication and rapport-building with members, families, and care teams.

Responsibilities

• Coordinate referrals to external providers to meet care needs.
• Document referrals accurately and maintain current records in the EMR.
• Build relationships with members and families to facilitate care coordination.
• Monitor care plans and follow up on goals with the care team.
