ArchWell Health is seeking a dedicated Referral Coordinator to guide members through the healthcare system, ensuring timely access to the right specialists and services. You will collaborate with a multidisciplinary care team, document referrals, and address barriers that may affect care delivery for seniors.

The ideal candidate has strong communication skills, a deep understanding of Medicare and insurance processes, and a passion for helping seniors navigate complex care plans.

Qualifications

• 2 years of experience in referral management or care coordination.
• Medical Assistant or medical administrative experience is helpful.
• Experience with EMR systems, ideally eClinicalWorks or similar.
• HS Diploma or equivalent is required; college coursework in healthcare preferred.

Responsibilities

• Coordinate referrals to external providers and services to ensure care needs are met.
• Complete proper documentation for member referrals to ensure records are accurate.
• Build and nurture relationships with members and families to facilitate care coordination.
• Be the system navigator and point of contact for members and families.
