ArchWell Health is seeking a Care Management Specialist to assist Nurse Care Managers and Chronic Disease Managers in coordinating patient care across our Florida clinics. You will support outreach, non-clinical needs, and medical records management to ensure seamless care transitions.

This role emphasizes strong organization, empathy, and teamwork, with duties including documentation in the EHR and collaboration with care teams. Travel up to 20% may be required.

Qualifications

• High school diploma or equivalent required.
• MA/LPN or CNA certification preferred.
• Minimum 5 years experience as MA/LPN/CNA, preferably in care management or acute care facility.
• Proficient PC skills and excellent customer service.
• Spanish or other languages spoken by communities served are desirable.

Responsabilités

• Outreach to patients to address non-clinical needs and coordinate care.
• Coordinate services such as follow-up visits, DME deliveries, home health initiation, and transportation.
• Collaborate with Nurse Care Managers, Chronic Disease Managers, providers, and center teams to support patients.
• Maintain accurate documentation on outreach, interventions, and outcomes in the EHR.
• Work with Care Management to meet departmental and organizational goals.
• Provide administrative support to the Nurse Care Manager and Chronic Disease Manager.
